Katherine Czarnowski, PharmD, BCACP

Katherine Czarnowski, PharmD, BCACP is a population health clinical pharmacist at Southern New England Health Organization (SoNE Health), a clinically integrated network in Connecticut. She works directly with primary care providers and patients to optimize medication regimens for chronic diseases. She also designs the strategy for Medicare star measures such as medication adherence, statin use in diabetes and statin use in cardiovascular disease. Previously, she spent four years at Cambridge Health Alliance in Massachusetts as an embedded ambulatory care pharmacist, practicing under a collaborative practice agreement for type 1 and type 2 diabetes, hypertension, hyperlipidemia, COPD, asthma, smoking cessation, travel medicine and anticoagulation management.
