Erick Sokn, PharmD, MS, presently serves as director of pharmacy for population health at Cleveland Clinic, with responsibility for personnel and strategic initiatives involving primary care, emergency medicine, transplant services, anticoagulation clinics, specialty clinics and transitional care services. He is responsible for leading pharmacy efforts to design care delivery models embedding pharmacists with interdisciplinary teams in multiple specialties to achieve quality goals through comprehensive medication management services.
Dr. Sokn is an active member of several pharmacy advisory groups focusing on pharmacist practice advancement, particularly in the space of ambulatory practice and comprehensive medication management.
