Practice Transformation: CMM Adoption Across the Care Continuum (1 hour)

The Practice and Care Delivery Transformation Workgroup leadership discusses the various regions of practice transformation that are necessary to advance an integrated comprehensive medication management (CMM) practice. Peer-reviewed research is presented to identify the current state of evidence, how to overcome gaps in CMM implementation and the different metrics used to evaluate a CMM program. Presenters discuss examples of best practices in both the public and the private sector as well as effective, compelling strategies to encourage implementation and adoption.

Learning Objectives:

  1. Determine the tools needed to address the current gaps in CMM implementation and research needed to work with health plans, employers and consumers.
  2. Identify the classification and utilization of metrics in a CMM practice setting.
  3. Showcase expert practices who have implemented successful CMM programs that optimize medication use through patient-centered, team-based services.
  4. Examine the difference CMM makes for a patient by decreasing waste, increasing quality and saving both lives and money.
