Payment and Policy: Demonstrating Value (30 minutes)

The Payment and Policy Solutions Workgroup leadership discusses the value and necessity of key players to facilitate buy-in of CMM as well as policy solutions and payment reform options for both advocates and policy makers. In addition, you’ll learn how telehealth provides a path to collaborative, interprofessional care teams by examining the current state and identifying solutions to key barriers to support expansion of CMM.

Learning Objectives: 

  1. Identify key stakeholder values to facilitate engagement and buy-in towards the adoption of CMM.
  2. Discuss a CMM value framework, with policy solutions and payment reform options for programs, designed to save lives and save money through a patient-centered, team-based approach.
  3. Learn how telehealth provides a path to interprofessional care teams working collaboratively to optimize medication use.
  4. Analyze the key barriers for telehealth delivery of CMM implementation and evaluate the solutions to support expansion of CMM.
