The new MedOpt-CKM Change Package, developed by pharmacists, provides tools to pave the path to an evidence-based focus on implementation of pharmacist-led comprehensive medication management (CMM) within a multiprofessional team to achieve medication optimization for persons with chronic kidney disease and cardiovascular-kidney-metabolic (CKM) syndrome. The MedOpt-CKM Change Package is designed to help pharmacists and implementation teams utilize CMM with fidelity for patients with CKM. The Change Package guides pharmacists new to CMM through the CMM Implementation System Pathway and introduces them, as well as pharmacists currently using CMM, to additional descriptions, experiences, quotes from patients, actions, and resources for each step in patients with CKM. The Change Package is meant to be a living document that will be updated over time. This effort was funded by the American College of Clinical Pharmacy (ACCP) Foundation.
