Webinar | A Better Way to Manage Medications

Prescription medicine is the most common treatment for medical problems in America, but medication has the power both to heal and harm.  Illness and death resulting from the wrong prescription, the wrong dose, skipped doses or new medication-related issues cost an estimated $528 billion each year.

HealthPartners is an integrated health care organization—serving more than 1.8 million medical and dental health plan members nationwide.

Comprehensive medication management is available as a benefit to all HealthPartners members with pharmacy benefits through HealthPartners (Medicare, Medicaid, Commercial) and is optional for employers who carve out their pharmacy benefits to an outside PBM. It is a benefit—no minimum criteria required for participation. High-risk candidates are targeted for services (invitations, financial engagement incentives for network providers) utilizing HealthPartners pharmacy and medical claims information and EHR data (when available) in a proprietary algorithm.

In this webinar, participants will learn about a team-based, patient centered approach to medication optimization proven to both improve patient care and reduce overall health care costs from GTMRx Distinguished Fellow Mary McClurg, PharmD, and Annie Ideker, MD.

Learning Outcomes:
After the webinar, participants will be able to:

  1. Describe the problem of medication non-optimization and its financial and clinical impact on client health and well-being;
  2. Distinguish between the common practice of medication therapy management and the specialized, clearly-defined practice of comprehensive medication management;
  3. Assess the evidence for the efficacy of comprehensive medication management as it achieves the quadruple aim of better care, lower costs, improved patient experience and improved provider well-being; and
  4. Identify the key barriers to wide-scale implementation of comprehensive medication management.
